Questionnaire

This is possibly the most important page of our entire "Portraits in Music" web site. In order for us to successfully complete a highly personalized musical portrait, we need you to give us as much detailed information as possible regarding the person for whom we'll be creating our portrait.

The attached questionnaire prompts you for all sorts of relevant information about your honoree. When filling out each field of the form, please feel free to be as longwinded as you'd like. We can always pare down details when converting your data into your song. But you don't really want us making things up or filling in the blanks ourselves. We naturally want you to recognize the person portrayed in the finished product.

Filling in the Form

If you consider any of the form's fields to be irrelevant or inappropriate, simply leave them blank. But don't scrimp on information in the fields you do deem appropriate. Also, be sure to let us know which details are imperative as well as which details are relatively minor.
